Login to the BACKEND control panel at www.yourchurchsite.com/administrator (where yourchurchsite.com is YOUR actual church domain name ie. www.crossroadschurch.com/administrator).
Go to components>donations>fields management
This page will list all the campaigns that people can give to.
Click on the New icon 
at the top right of page.
Lets add a field called "Do you attend our church regularly?"
So lets fill out the following fields this way:
Name: Regular
Title: Do you go to our church regularly?
Field Type: Radio List
Description: leave blank
Required: this is up to you if you want it to be required or not
Values: Yes No
Default Values: leave blank
Click Save.
